The Vital Statistics office has on file birth certificates from 1911 to present. This office has no records of births occurring prior to the above date except delayed records of births for persons born before 1911 which have been established by affidavits and documentary evidence. You can purchase a certified birth certificate by phone, by mail, off the Internet, or by visiting the office. See purchase options details on Vital page. Please allow up to approximately 30 working days for processing of all type of certificates when ordered through the mail.
NOTE: The Kentucky Office of Vital Statistics cannot by law, provide free information from records, do a genealogical search, or look to see if a certificate is on file without a completed application form and the proper fee attached. The Kentucky Vital Statistics office will, upon receiving the application and fee, do a search of the records and if a certificate is on file, it will be mailed to you. The fee is for the search of the records and no refunds will be issued if a certificate can not be found. The office does not issue un-certified or photo copies of certificates.
Information needed to obtain a certificate:
• Full name at birth of the person for whom the certificate is requested
•The month, day and year of birth
•The county of birth
•The mother's maiden name
•The father's name
Additional information needed:
•Name and address where the certificate is to be mailed.
•Phone number where you can be reached during the day from 8:00-4:30 Eastern Time.
•The number of copies being ordered.
